Start Date: 11/11/2015Start Time: 12:15 PM
End Date: 11/11/2015End Time: 1:15 PM
Event Description

Dr. Shreya Kangovi grew up in India, where she observed lay people doing medical outreach work, going to homes to help the underprivileged with health care issues. Seeing these "community health workers" in India, Dr. Kangovi realized the United States could apply this same approach in needy communities. This was her inspiration for the Patient-Centered Transition (PaCT) project, in which community residents help keep underprivileged patients on course after a hospitalization. In this video Dr. Kangovi recounts the case of a patient who was able to turn his life around with the assistance of one of these "natural helpers."
