Title:
A Road to Performance Improvement and Beyond: Optimizing Early Pregnancy Failure Management in Emergency Departments

Speaker:
Elizabeth Datner, MD, FACEP
Chair, Department of Emergency Medicine
Einstein Healthcare Network
Professor of Emergency Medicine
Sidney Kimmel Medical College
Thomas Jefferson University

Details:
One in four (1 in 4) women experience miscarriages in their lifetime, and many of those women seek care in local emergency departments, while 1.6% of emergency department visits (about 500,000) in the US are for patient complaints of bleeding during the first trimester of pregnancy. About 25% of these women will be diagnosed with a miscarriage. Traditionally, patients have been treated for miscarriage through one of three ways: expectant management, medical management, or surgical uterine evacuation. Surgical evacuation carries minor risks, takes significant time for surgeon and operating room availability, and has significant associated costs. A method for manual uterine aspiration without the need for operating room availability has advantages. The procedure is not yet broadly available in emergency departments for various reasons, including the need for provider training in the technique.

To improve the availability of the manual uterine aspiration technique for treatment of early pregnancy loss (miscarriage), providers in emergency departments and other specialties need to have the opportunity to learn and practice the technique. In collaboration with the Drexel University School of Biomedical Engineering, Science and Health Systems, we are developing a simulation model that will allow clinical providers to learn this important technique and bring it to the patients who need this important procedure.

Biosketch:
Elizabeth Datner, MD, FACEP, currently serves as the Chair of the Department of Emergency Medicine at Albert Einstein Healthcare Network in Philadelphia, PA. Dr. Datner is a Professor of Emergency Medicine at the Sidney Kimmel Medical College of Thomas Jefferson University. She joined Einstein in November of 2016. Prior to that time, Dr. Datner served as the Chief of Staff at the Corporal Michael J. Crescenz VA Medical Center in Philadelphia, PA, and was on the faculty and a member of the Department of Emergency Medicine at the Ruth and Ray Perelman School of Medicine at the University of Pennsylvania.

Dr. Datner’s academic interests focus on providing improved care to underserved populations; specifically addressing improving the operational effectiveness of hospitals and emergency departments, improving the quality and safety of care in a hospital or emergency setting, and specifically addressing issues of interpersonal violence and women’s health. Her work has explored solutions to emergency department overcrowding and developing innovative methods to alleviate crowding while providing high quality care. Dr. Datner has worked to improve identification of youth and women at risk for interpersonal violence, while developing interventions to assist these at-risk groups. In addition, she has pursued quality improvement through the development of innovative operational, social service, and multidisciplinary programs.

Dr. Datner's work has previously been supported by both federal and foundation funding. She has served on state and city-wide committees to address interpersonal youth and domestic violence concerns. Dr. Datner has also served on several Boards of Directors, including Physicians for Social Responsibility, Philadelphia Research and Education Foundation, and the Legal Help Center in Philadelphia, PA. She is a recipient of the Gold Humanism in Medicine award and is a member of the Gold Honor Society.
