About
Schedule your appointment at our sign up table and we will take care of the rest. We will focus on obtaining your script and scheduling your preferred time slot while you focus on you. Our mission is to make your annual screening a fun and hassle free experience.
Prior to scheduling your appointment, please keep the following details in mind:
- You must be 40 years or older.
- It must be at least a full year since your last screening mammogram.
- Please bring photo identification, your insurance card and physician contact information
- If you plan on attending Employee Mammogram Day, please let your supervisor know in advance.
- Depending on your insurance, co-pays or deductibles may apply.
Refreshments and giveaways will be provided after your screening! |
