Event Description
The Office of Public Health Practice presents Gladys Antelo-Allen, LPN, program manager at the Camden Coalition of Healthcare Providers.
 
Learn about how student hotspotting engages interdisciplinary teams of 4-6 students partnering with a local hospital to identify a small caseload of patients (3 or 4) who are super-utilizers of healthcare services. Students work with patients to create a care plan and provide support in navigating social services and attending medical appointments.
